MCCI is seeking a compassionate Care Partner to join our Care Services Team for a maternity leave contract, supporting older people to live independently and with dignity in their own homes. You will work across Fyshwick and offsite locations, delivering person-centred care and culturally responsive practice.

Responsibilities include assessing needs, developing care plans, coordinating with clients, families and providers, navigating aged care systems, monitoring delivery, and maintaining
