About The Opportunity
COPILOT is a dynamic and exciting service that provides rapid access to multi-disciplinary geriatric assessment (medical, nursing, and allied health) for older people living in the community setting. Referral enables care to be implemented in the patient's home environment, whether that be in RACH's or their own homes, avoiding unnecessary transfer to hospital and leading to improved patient outcomes.
The COPILOT VMO will provide a high quality clinical service to older people and their families/carers to optimise health outcomes and support clients keeping well in the community. The VMO will provide comprehensive geriatric assessments and evidence-based care to clients residing in their own homes or nursing homes for acute care with the goal of avoiding unnecessary transfer to hospital.
The COPILOT VMO will need strong clinical leadership and decision making skills, have relevant clinical experience in the Aged Care speciality, and preferentially experience in providing community-based care both in a direct and virtual capacity. The position requires compliance with mandatory clinical administration duties, and participation in an on-call roster. The successful candidate should have strong communication skills, be efficient and have excellent time-management skills, and a willingness to join a supportive and dynamic multidisciplinary district-wide service.

What You'll be doing
Provide clinical management and treatment of patients in a SWSLHD facility under your care. This care will be provided within the clinical privileges granted by the CE on the advice of the SWSLHD Medical and Dental Appointments Advisory Committee.

About the opportunityThe Geriatric Evaluation and Management and Unit consists of a state of the art 30 bed inpatient ward with GEM/ TCP/ Rehabilitation that provides specialist in-patient care to the growing communities of the West. The Unit is based at Werribee Mercy Hospital. The unit is supervised by a Head of Unit and is made up of Geriatricians, an Advanced Trainee, BPT registrar, HMO's and an Intern. There is a strong multidisciplinary team consisting of medical, nursing and allied health staff. In addition to ward responsibilities, you will have the opportunity to be involved in ACAS rounds as well as specialist clinics. Werribee Mercy Hospital is a teaching hospital of the University of Notre Dame.

An exciting opportunity to join the Geriatrics team at both the Mona Vale and Northern Beaches Hospital.
To provide effective clinical management of patients, under appropriate supervision, including patient assessment, making treatment decisions, performing procedures and maintaining adequate medical records; and to develop expertise in medical practice, in a complex clinical environment.
The position is accredited for 24 months of training and offers 6-month rotations through Geriatric Rehabilitation and Community Rapid Access (BRACE) at Mona Vale Hospital and 6 months rotations in Orthogeriatrics and Acute Geriatrics at the Northern Beaches Hospital.
Mona Vale Hospital is a subacute facility providing the following services:
24 bed Assessment and Rehabilitation Unit (ARU) for Geriatric Rehabilitation
26 bed Beachside Rehabilitation Unit (BRU) for General Rehabilitation, Stroke and Amputees
20 bed Palliative Care and Geriatric Assessment and Management Unit
24/7 Urgent Care Centre, treating patients with minor injury and illnesses
Community Health CentreThe hospital is located in a beautiful beachside setting, is accessible via Public Transport or alternately offers free parking for those using their own transport and is close to shopping centres and other community / social facilities.
Northern Beaches Hospital provides world-class, Level 5 delineation services for both public and private patients. The hospital features 486 beds, 50-bed Emergency Department, 20-bed general and cardiothoracic ICU, 14 fully equipped theatres, including hybrid theatres, two cardiac catheter labs, extensive digital imaging / diagnostic facilities, medical centre co-located with ED, state of the art education and interactive SIM centre and a Helipad for Emergency Transport.
The Northern Beaches Hospital provides a high standard of education and training, with dedicated supervisors, teachers and a JMO Unit that supports all activities related to rostering, training, education and welfare of junior doctors.
On level 4 of the 9-storey hospital is the Education Centre where doctors can access a range of facilities. These include the latest technology for teaching, simulation and presentations, plus various meeting rooms and conference areas.
There is capability to stream education sessions to/from other facilities, as well as within the hospital for the simulation rooms and theatres.

The Role
The Team Leader, Home Care Queensland is responsible and accountable for the leadership, delivery and continuous improvement of high-quality in-home services for all Support at Home Packages as well as Veteran Home Care (VHC) funded services. What We're Looking For Key Result Areas
- Ensure delivery of high-quality, person-centred care aligned with Support at Home and VHC program requirements
- Maintain accurate, up-to-date client documentation (care plans, reviews, progress notes)
- Lead, coach, and support SAH Care Partners and Veterans Home Care Coordinator
- Oversee intake processes for Support at Home, and VHC clients
- Monitor timeliness of onboarding, assessments, and care plan development
- Ensure accurate data entry and client profile creation in the client management system
- Oversee clients' quarterly budgets to ensure SAH services do not exceed budget
Qualifications
- Tertiary qualification in Social Work, Nursing, Aged Care, Disability, Allied Health, Business Management, or a related field. Current registrations as required i.e. AHPRA Registration.
- Qualification in management and leadership, or equivalent workplace experiencewould be highly regarded.
Experience
- Demonstrated leadership experience in aged care, community services, or a related human services field.
- Demonstrated knowledge and sound case management experience.
- Clear understanding of the Aged Care Quality Standards
- A working understanding of health and safety requirements relevant to the position
- Solid knowledge of the Support at Home and Veterans Home Care program rules and guidelines.
